The most common classroom behavior chart is on a wall, with clips or cards or colors, and every child can see where every other child sits on it all day.
That is a design decision, and it is worth noticing that it was made for you rather than by you. A wall chart makes a hard morning public, permanent until home time, and legible to any parent who walks in at pickup.
This one sits in a folder on your desk. It does the same job, keeping a record you can point to in a conference or a phone call, without a child spending the afternoon looking at proof that everyone knows.
A hard Tuesday should not still be on the wall on Friday.

The marking key at the bottom is four levels, and the wording is deliberately plain. If your school uses different words, the pass lets you type your own.
Most difficult conversations with a family go better with a specific week in front of you. Not a colour or a mood, but four Tuesdays and what happened on each.
Keep the sheets in a folder by grading period and the record builds itself. You will use it twice a year and be glad both times.
