Nobody fills in sub plans on a calm Sunday. They get filled in at six in the morning, one-handed, by somebody who has already decided they are too sick to drive.
So every line here can be answered in a few words. Times, subjects, and one short instruction each. There is no box asking what your learning objective is, because at six in the morning the honest answer is "keep them busy and safe until I am back," and a page that will not accept that answer just does not get filled in.
A blank page is the most common set of sub plans in America. This one is trying to beat blank.
Students who can help. Every class has a child who knows the routine better than any adult in the building. Naming three of them saves a substitute an hour.
Adults nearby. Next door, team lead, front office. A substitute who does not know who to ask will not ask.

Print two or three copies with those parts already filled in and leave them in a folder on your desk. On the bad morning all you add is the date and the schedule.
